Dear Prospective Vendor,

We are very much interested in learning more about your qualifications for installations and/or field service work. In order to use your company as a part of our service network, and to assist us in determining when and how your company can best service our client’s needs; please complete the attached data package supplying all pertinent information.

It is very important that you sign the attached federal tax ID# form identifying either your federal tax number or social security number. If your work is confined to a particular region, then please also supply us with your territory map. We would be interested in any information or brochures about your company, as well as your service and installation pricing.

As we strive to maintain the highest image and customer service standards in the industry we would ask no less than the same of your company. Since your company is our front line contact with our customers, we also require photos of each of your crew(s) with their equipment. We would appreciate your photos in a jpeg format if possible. Before purchase orders are issued, or work assigned, we require a certificate of insurance be issued, on our behalf, listing SignResource as an additional insured. Please have your insurance company email this certificate to bcox@signresource.com, following up with the original in the mail:

SignResource minimum insurance requirements are as follows:

• General Liability Insurance
  • $1,000,000 per occurrence/with a $1,000,000 general aggregate
• Automobile Liability Coverage (not physical damage coverage)
  • $1,000,000 CSL (Combined Single Limit)
• Workers Compensation Insurance
  • Coverage A – Statutory Limit as required by your state
  • Coverage B – Employers Liability $500,000
    (If your employees are leased, then we also request that an alternate employer endorsement be attached)
• Prior to hiring, SignResource would need to be listed as an additional insured and be given a copy of the COI.
